Description: What Parents Need to Know About Your Child’s Transition to Adult Health Care.  Helping your child transition to adult health care is a big step for any parent. It involves encouraging them to become more knowledgeable about their health and more independent in their care, from filling out their own forms to scheduling their own appointments. Got Transition is a national organization based in DC that works to help young adults and their families with this transition. In this session, we will offer a quick overview of health care transition, a few helpful tips to encourage your child to become more independent, and some easy tools to help your child take charge of their health.

Got Transition is a national organization based in DC who works to help young adults and their families with this transition. In this session, we will offer a quick overview of health care transition, explain the importance of health as it relates to independent living and post-secondary life, and offer some tools for special educators to use in the transition plan of the IEP (Readiness Assessment and Sample Goals).
