Thank you for emailing the previous documents. I am writing to request my child, ___________’s (DOB:_____) complete and cumulative educational records, including but not limited to:

• All school meeting notes (MDT meetings, IEP meetings, eligibility meetings, meetings to address bullying concerns, parent-teacher meetings, etc.…);
• All standardized test scores  
• All progress reports 
• All class schedules;
• Bullying Investigation Reports, including the point of contact for the investigation, the conclusions and the recommendations for next steps;
• School’s bullying policies and procedures;
• School-wide bullying prevention measures;
• School nurse records for my child;  
• Incident Reports connected to the reported bullying incidents my child experienced at school;
• Witness statements from students (redacted), teachers, administrators, or other personnel from the bullying investigations; 
• Student statements to the investigators, teachers, etc… (redacted);
• My child’s statements to the investigators, teachers, etc.…
• Police Reports and contact information for the police in charge of investigating the bullying 
• Formal bullying complaints and reports I submitted, i.e. documents/details parent provided to the school and/or the CARE Team, or another agency;
• Safety Plans or Crisis Plans;
• Bullying prevention plans;
• All evaluations/assessments;
• All academic records (report cards, progress reports, course test scores, daily schedule);
• All communication charts/logs; 
• Parent Contact Log;
• All observation reports about my child;
• All social-emotional records, etc.…, and 
• Index of all the records in my child’s student file

I would like copies of all these documents as PDF attachments.  

Please confirm receipt and let me know if you have any questions. Please let me know if you will be able to fulfill any part of, or the entire, request for records by ___________. They can be (emailed or mailed) to me at this address ________________ (or I can pick them up from the school).
