  • Behavior trackers
  • Suspension notices/letters – describing the behavior/reason for suspension, the number of days, who made the decision, etc…

• Incident reports and/or investigations
• Evidence of alleged incident(s) – anecdotal, photos, video footage, witness statements/interviews, etc…
• Behavior plans, 
• Behavior contracts, 
• Safety/Crisis Plans, 
• Behavioral evaluations, 
• Descriptions of the Responses to Intervention (“RTI”) the school tried,
• List of trusted adults in the school, etc…

• School meeting notes written by the parent/guardian(s) and/or the school, i.e., MDT meetings, meetings about social-emotional-behavioral concerns, suspension conferences, parent-teacher meetings, etc…
• Medical records:
  • Prescriptions
  • Documents confirming diagnoses
  • Intake documents
  • Doctor notes excusing absences 
  • Information about outpatient doctors, like a therapist or psychiatrist? If yes, what are their names, why does she see them, what is her treatment plan
  • Documents about any hospitalizations, (i,e. reasons for admission, length of stay, treatment plans)
• All Report Cards 
• Parent-Contact Log
• Class schedules
• All attendance records, (i.e: daily attendance, attendance for each class, an explanation for all abbreviations/codes in the attendance record; attendance incentive plans, attendance incentive plan, etc…)
• Math, reading comprehension, and writing schoolwork samples;
• All observation reports; and
• Parent Contact Log or other communication logs;   

I would like copies of all these documents as PDF attachments.
