- School correspondence to and from school staff, such as: texts, including screenshots, emails, letters, etc.…)
- All school meeting notes (MDT Meetings, IEP/504 Plan meetings, eligibility meetings, manifestation determination meetings, behavior meetings, parent-teacher meetings, attendance meetings, etc.…);
- All involuntary transfer documents, i.e.
- Parent-Contact Log about the involuntary transfer
- Copies of emails and attachment sent to parent regarding the involuntary transfer
- Copy of the involuntary transfer procedures emailed to the parent
- The administrative statement,
- Notice of Involuntary Transfer,
- Records DCPS is relying on when issuing the proposed involuntary transfer,
- Administrative conference invitation,
- Administrative conference meeting notes,
- A letter with a proposed school location/placement,
- A letter of invitation to a multi-disciplinary (MDT) onboarding meeting,
- DCPS notes from onboarding meeting,
- Investigation documents,
- Staff interviews and statements,
- Interview of the student that may be involuntarily transferred,
- Redacted student interviews and statements,
- Evidence of other attempted interventions before involuntarily transferring the student,
- Documentation of factors considered when selecting the proposed new location of service, etc…
- All school incident investigation reports and documents for staff misconduct;
- All IEPs;
- All 504 Plans;
- All Service Trackers;
- All IEP/504 Progress Reports;
- All eligibility determination documents, including requests for supports under a 504 Plan;
- All evaluations/assessments (For ex: Psychoeducational, psychological, Neuropsychological, screeners, any independent evaluations, Analysis of Existing Data (“AED”) Reports);
- Documents related to evaluations (Any written requests to evaluate, School’s confirmed receipt/acknowledgement of parent/guardian requests to evaluate, Evaluation Consent Forms, Referral for Evaluation, etc.…);
- All Prior Written Notices (‘PWN”);
- All Letters of Invitation (“LOI”);
- All academic records (report cards, transcripts, progress reports, etc.…);
- All course schedules, including when my child received related services, including transition services;
- All standardized testing results and reports (The summative assessments for each class and Statewide, i.e. iReady, PARCC, MAPP, etc.…)
- All administrative documents (enrollment forms, Student/Family Handbook, School Calendar, etc.…)
- All student support team documents,
- All social-emotional-behavioral and discipline records, including:
- Incident reports,
- Behavior trackers,
- Records of early dismissals, where the school asked the parent to pick the child up early – not including for medical reasons,
- Suspension notices/letters,
- Witness statements for alleged incidents,
- Evidence of alleged incident – anecdotal, photos, video footage
- Behavior plans,
- Behavior contracts,
- Behavioral evaluations,
- Staff interviews and statements,
- Redacted student interviews and statements,
- Documentation/descriptions of the attempted Responses to Intervention (“RTI”),
- Documentation/descriptions of the attempted Multi-Tiered System of Supports (“MTSS”) and other student support services (“SST”) attempted
- Documentation/descriptions of the attempted restorative practices staff implemented
- Records and investigations about concerns about bullying,
- All attendance records, (i.e.: daily attendance, attendance for each class, an explanation for all abbreviations/codes in the attendance record; attendance incentive plans, etc.…)
- All ESY and/or summer school documents;
- Parent Contact Log or other communication logs; and
- Math, reading comprehension, and writing schoolwork samples from this school year and the prior two school years;
- All observation reports;
- All transportation records (i.e., transportation request forms and transportation eligibility documents);
- PowerSchool Index, IEP Event Page, or other index of all the records in my child’s student file
I would like copies of all these documents as PDF attachments.
