Pursuant to The Family Educational Rights and Privacy Act or FERPA (20 U.S.C. § 1232g; 34 CFR Part 99) and The Freedom of Information Act (D.C. Code §§ 2-531-539), I am writing to request my child, ___________’s (DOB:_____) complete and cumulative transportation records for the following school years _________, ____________, including but not limited to:

• Transportation Request Forms (“TRFs”)
• Route records, i.e.:

• Route/bus number
• Names of drivers and aides
• Qualifications of assigned Division of Transportation (“DOT”) staff, including training DOT staff received before OSSE assigned them to this route. 
• Number of students on student’s route 
• Distance of commute 
• How long each part of the commute is, i.e. how many minutes, hours, etc…?
• Type(s) of vehicle(s)
• Climate control features; is there AC, heating, ability to open windows?
• Bus tickets
• Scheduled and actual pick-up and drop-off times, pick-up and drop-off locations
• Route tracker – actual pick-up and drop-off times and locations
• Audio recordings of phone calls with The Parent Resource Center
• OSSE DOT Database Index aka IDS Index of Documents for Student 
• Incident reports
• Notes from bus driver and aide 
• Correspondence with OSSE Division of Transportation/OSSE in general via email, text messages, and letters
• Audio Recordings of Parent Resource Center Calls. 

• Parent Contact Log or other communication logs;  
• Transportation Record Index, and 
• OSSE Division of Transportation agency records. 

I am submitting this letter along with the complete Education Records Disclosure Authorization Form and a photo of my ID card.
