The illustrated cover has a light lavender background and a large white, torn-paper-style panel. The AJE logo appears near the top. Colorful books, pencils, paintbrushes, an apple, alarm clock, globe, backpacks, stars, and school supplies frame the title, “Back-to-School Health Checklists by Age,” with the message, “Ready, Set, Grow!”
