This quote came from a participant in Dc Health Matter’s 2019 Community Health Needs Assessment for the District of Columbia. Thier conversations with over 300+ community stakeholders resulted in a rich collection of 28 top themes in four areas: Mental Health, Care Coordination, Health Literacy and Place-Based Care.
