Health Literacy and Place-Based Care in DC webinar

“For place-based care and health literacy, we are not thinking creatively enough or boldly enough.”

This quote came from a participant in Dc Health Matter’s  2019 Community Health Needs Assessment for the District of Columbia. Thier conversations with over 300+ community stakeholders resulted in a rich collection of 28 top themes in four areas: Mental Health, Care Coordination, Health Literacy and Place-Based Care. (The full report is available on DCHealthMatters.org.)

The DC Health Matters Collaborative will share more about what DC wants to see the health system tackle related to Health Literacy and Place-Based Care in the last webinar in their four-part series on Thursday, Nov 14 at 2 pm.

A discussion of their assessment findings and examples of our initiatives will be presenters by representatives from Howard University Hospital, Mary’s Center and Sibley Memorial Hospital.
