Don’t Forgot – Students still need to get all their Required Immunizations!

Is your child up-to-date on all of their required immunizations to attend school in-person?   If not, please take these four steps now:

  1. Schedule your child’s annual wellness visit today and ask your physician to complete the Universal Health Certificate. To make an appointment, call your doctor’s office or find a provider by clicking here. If you need access to health insurance, please visit the DC Health Link website or call 311.
  2. Request a copy of your child’s immunization history from your doctor to share with the school.
  3. Submit the Universal Health Certificate and any other immunization documents to your child’s school.
  4. Contact your school to confirm they’ve received it.
