Three Trainings about Health Care Transition

AJE is happy to share these resources from the National Alliance to Advance Adolescent Health and Got Transition about the transition to adult health care! These recordings were made by AJE as part of SchoolTalk’s 2020 Voices of Change conference on Secondary Transition.  The National Alliance to Advance Adolescent Health is a nonprofit organization whose mission is to achieve long-term, systemic improvements in comprehensive health care and insurance coverage for adolescents. The organization works to promote effective transitions from pediatric to adult health care as part of its Got Transition program, the federally-funded national resource center on health care transition.

 

What Parents Need to Know About Your Child’s Transition to Adult Health Care.  Helping your child transition to adult health care is a big step for any parent. It involves encouraging them to become more knowledgeable about their health and more independent in their care, from filling out their own forms to scheduling their own appointments. Got Transition is a national organization based in DC that works to help young adults and their families with this transition. In this session, we will offer a quick overview of health care transition, a few helpful tips to encourage your child to become more independent, and some easy tools to help your child take charge of their health.  Resources for Parents

The Role of Special Educators and Transition Coordinators in Supporting Health Care Transition for Students.  Transitioning to adult health care is a major step for all students, and especially for those with special health care needs. Got Transition is a national organization based in DC who works to help young adults and their families with this transition. In this session, we will offer a quick overview of health care transition, explain the importance of health as it relates to independent living and post-secondary life, and offer some tools for special educators to use in the transition plan of the IEP (Readiness Assessment and Sample Goals).
